Community Health Nursing

Our Community Health nurses support people living in the community to manage their health conditions by providing individual health assessments, health information, education on managing a health condition, and lifestyle assessments.

Our qualified staff provide support to people seeking assistance with:

• asthma, including education for groups and individuals on managing the condition
• managing a long-term illness or condition. by providing support, education and referral options for your relevant condition and aimed to improve quality of life
• female sexual and reproductive health. The Well Women’s clinic provides education and support and cervical screenings (PAP screening)
• neurological disorders such as Parkinson’s disease, Multiple Sclerosis, Motor neurone disease and supports people living at home and in aged care to link to appropriate services, and maintain independence and wellbeing
