Care Management – Support at Home

To access Support at Home you need to have an aged care assessment. You can apply for an assessment online on the My Aged Care website at the link below. You can complete the application for yourself or have someone you trust do it for you like a family member or friend.

You can choose what organisation you want to provide your aged care services. Grampians Community Health is a registered provider of the Support at Home program. People who have been approved for Support at Home services can request Grampians Community Health as their provider.

Care Management is available Monday to Friday 9am to 5pm.

Support services as listed above can be provided at a time that meets the need of the package holder and/or their carer.

Under Support at Home, your income and assets will be assessed to work out how much contribution you have to make.

When you get Support at Home services, 10% of your budget will go towards Care Management.

Your budget depends on your assessed care needs. It includes government funding and your contributions.

Support at Home costs and contributions | My Aged Care 

You may be eligible for funded aged care services under Support at Home if you:

• are 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people) and
• have noticed a change in what you can do or remember
• have been diagnosed with a medical condition or reduced mobility
• have experienced a change in family care arrangements
• have experienced a recent fall or hospital admission.
