The Support at Home Program started in November 2025. It replaced the Home Care Packages Program and the Short-term Restorative Care Programme.
Care Management is a service older people receive as part of ongoing care through the Support at Home program. A Care Partner ensures you get the right care for your preferences, culture, and needs, including end-of-life care. A Care Partner helps with care planning, coordinating, reviewing, and monitoring of your services. Their role is to help you understand and access the services and care you need. They can also help with advocacy, education and support.
The new Statement of Aged Care Rights outlines the rights that older people have when accessing aged care services. The rights make sure older people and their needs are at the centre of the new aged care system. Learn more by watching this short video:

To access Support at Home you need to have an aged care assessment. You can apply for an assessment online on the My Aged Care website at the link below. You can complete the application for yourself or have someone you trust do it for you like a family member or friend.
You can choose what organisation you want to provide your aged care services. Grampians Community Health is a registered provider of the Support at Home program. People who have been approved for Support at Home services can request Grampians Community Health as their provider.
Care Management is available Monday to Friday 9am to 5pm.
Support services as listed above can be provided at a time that meets the need of the package holder and/or their carer.
Under Support at Home, your income and assets will be assessed to work out how much contribution you have to make.
When you get Support at Home services, 10% of your budget will go towards Care Management.
Your budget depends on your assessed care needs. It includes government funding and your contributions.
