Grampians Mental Health and Wellbeing Connect is delivered in partnership between Mind Australia, Ballarat Community Health and Grampians Community Health.
Grampians Mental Health and Wellbeing Connect provides a warm and welcoming space for family, carers, kin and supporters of all ages and backgrounds.
The service provides opportunities to connect with peers along with tailored information, resources, advocacy, support (individual and group-based), and access to brokerage funds through Tandem’s Carer Support Fund.
At Grampians Mental Health and Wellbeing Connect, all staff have a lived experience of supporting someone with mental health and wellbeing concerns and/or substance use challenges.
Our family and carer peer support workers listen to families, carers, kin and supporters, and offer a variety of supports based on their specific needs.
Supports for family members and carers may include emotional or practical support and help to connect with information, resources and services relevant to their situation.

Grampians Mental Health and Wellbeing Connect is for family members, carers, kin and supporters of all ages and backgrounds who are living in the Ararat, Ballarat. Golden Plains, Hepburn, Hindmarsh, Horsham, Moorabool, Northern Grampians, Pyrenees, West Wimmera, and Yarriambiack local government areas.
