Our Social Support Group program is a Community Access support service to engage people in group activities which draw on:
- Personal strengths
- Encourage independence
- Social interaction
- Involve you in your community
- Build capacityThe positive, supported environment provides social opportunities for participants and their carers to prolong independent community living. The progress and care of all participants within our social group setting is planned and monitored by group facilitators and support coordination staff. The social activities are developed around the interests and skills of the participating group members. Social Groups operate during the day and evening.
These social groups are funded from:
- Commonwealth Home Support Program (CHSP, people aged over 65) this service supports frail older people living in the community to maximise their independence
- VicHACC Program for Younger People (HACC PYP, people aged under 65) provides basic support and maintenance services to help people with disabilities remain active and connected.
Fees may apply- please enquire on referral.
