Women moving into the area are always welcome and the program can assist them to get to know other local women as well as providing other supports.
The GCH Aboriginal Primary Health Worker can help people with their health management including care planning, attending appointments, accessing aged care support (over 55 years) and seeking help from other health services and professionals. Often assistance is needed for other aspects of life including family matters, living arrangements, education, personal wellbeing and social connections. The GCH Aboriginal Primary Health Worker can also help find the right service and support as required by each individual.
Program participants meet regularly for social activities in Halls Gap and newcomers are always welcome.
The Aboriginal Primary Health Worker is based at the Stawell Health and Community Centre but can provide an outreach service to eligible people living in Ararat Rural City and Northern Grampians Shire.
Monday to Friday 9 to 5pm or by arrangement.
