The National Disability Insurance Scheme (NDIS) is the new way of providing individualised support for people with a disability. Grampians Community Health is an NDIS provider. When you are working with the National Disability Insurance Agency (NDIA) on your support plan, you can nominate Grampians Community Health as your service provider to partake in our Younger Active Persons (YAP) group and more.
Grampians Community Health provides (18 – 30 year olds) recreational and social experiences for younger people with a disability (including Autism).
Activities are initiated by group participants that reflect their personal interests and aspirations as well as developing cohesion, trust and social engagement.

The progress and care of all group participants is planned and monitored by group facilitators and support coordination staff. The social activities are developed around the interests and skills of the participating group members.
