Grampians Community Health are pleased to be able to offer Community Paramedic visits in our local region. These wonderful clinics are coming to rural Victorian towns, on country, and in your local venues.
They are a free, drop in service where we can provide support with your health and wellbeing.
We offer things like:
Identifying the risk of chronic health issues, and supporting people to manage their health problems
Providing assistance in navigating Health Care and Referrals.
Blood Pressure and Health Checks
Discussing medications or health problems
Health education, promotion and helping people live an active healthy life as you age.
And most important of all, keeping people well, happy and out of hospital and living their best life.
